Your new patient appointment typically lasts about 60 minutes. That time includes completing any remaining paperwork, a physician interview to review your symptoms and history, and imaging such as X-rays if needed. Please arrive 10 to 15 minutes early to allow time for check-in.
What to bring:
You can complete your new patient paperwork before your visit through our secure Patient Portal. If you are accessing the portal for the first time, start with My Health & My Forms — even if you have visited our office before.

A few things to know about your insurance coverage:
If you need to cancel or reschedule, we ask for at least 24 hours’ notice. This allows us to offer the time to another patient who may be waiting for care. To cancel, call 708.226.3300.
If you arrive late, we will do our best to accommodate you. However, late arrivals may result in a shortened appointment or a request to reschedule, depending on the schedule. Arriving 10 to 15 minutes early is always the best approach.
Our physicians are on call 24 hours a day, 7 days a week. For after-hours emergencies, call our main number at 708.226.3300 and follow the prompts. Prescription refills are handled during regular office hours only.
